Please select "I agree" to the below contract items.
 1.I agree that I can show the film during the date(s) specified above. Arrangements may be made should I wish to extend the dates for my showings by contacting marketing@monterymedia.com.
 2.I understand that private showings (for family, friends, other organizations, etc) would violate this distribution agreement and agree to no private showings.
 3.I agree to contact my own ISP (Internet Service Provider) to make certain I can receive important email communication from the domain montereymedia.com
 4.I will submit the dates of all showings to monterey media, inc. If any of the dates change from what I originally submitted or I add additional showings, I agree to notify monterey media, inc. by contacting marketing@monterymedia.com
 5.Once my event is posted on the website, I agree to check my information for accuracy and to request necessary changes by contacting marketing@monterymedia.com
 6.I agree that once I have been listed on the website there will be a $75 cancellation fee up to two weeks prior to my showing. Cancellations made within two weeks of the showing will be charged the $ minimum fee.
 7.I agree to suggest a minimum price of $10.00 per adult and $7.00 per student.
 8.I commit to pay 50% of receipts from ticket sales, with a minimum of $, for showings of the film . I understand that I will receive a payment envelope and invoice to be used in sending in my payment.
 9.I agree that I am responsible for all costs associated with the showing. This includes equipment, complementary tickets to volunteers, any advertising, tax, etc. I agree that any such costs shall come out of our portion of the proceeds and that monterey media, inc. will receive 50% of all actual ticket monies received.
 10.Payment is due one week after my showings. Payment for the Film: Payment can be made via Check, Money Order or Credit Card. For Checks, please send and make Checks or Money Orders payable to: monterey media, inc., 566 St. Charles Drive, Thousand Oaks, CA 91360. For Credit Card payments please NOTE: Credit card payments will be listed as monterey media, inc. on your statement.
 11.I understand there will be a late fee of $25 if my payment is not been received by 10 days after my last screening date.
 12.I agree to return both copies of the DVD for the screening within ten (10) days of my final screening. Failure to do so will result in a $200 penalty. I agree to get a tracking number from the courier or postal service to insure delivery of the DVDs and payment.

 13.I understand that monterey media, inc. is the exclusive US distribution company for the film.
 14.I agree that I do not have the rights to make any copies of the DVD whatsoever and will not do so. Making illegal copies of the film is a Copyright infringement and disrespectful to the filmmakers.
 15.No audio or video recordings of the movie will be allowed at any of my showings and I agree to take measures to enforce this.
 16.I can expect to receive the DVD event package no later than 1 week prior to my showing. Complete instructions and a return address will be in my DVD package along with a back up DVD just in case there are any issues with the first copy.
 17.I agree to test the DVD on the equipment that I will be responsible for arranging and using at least four days prior to my screening. Projection needs could entail a digital projector, DVD player, and large screen.
 18.I understand that any promotional information and materials provided by monterey media including the poster are only to be used in connection with publicizing the screening.
 19.I understand and agree that by submitting this contract and agreeing to the above terms and conditions, this becomes a valid agreement between monterey media, inc. and myself/organization.
 

For Canadian Hosts:
 20.I agree to pay monterey media, inc. $5.00 US dollars per ticket sold or otherwise issued. I am responsible for adjusting my ticket sales price to make up the difference in the currency exchange rate.
 21.The DVD will arrive in a Region One specific format, which is compatible with new international DVD players (as well as standard US DVD players).
